Do I have a deductible and, if so, has it been met?
How many sessions per year does my plan cover?
Do I have a co-payment and, if so, how much is it?
What is the coverage amount for out-of-network providers?
Is preapproval required from my primary care physician?
Once an appointment is scheduled, you are responsible for full payment unless you provide more than 24 hours advance notice of cancellation or rescheduling. Insurance does not reimburse for no-show/late cancellation fees.
